What Is Breast Augmentation?

Breast augmentation is one of the most commonly performed cosmetic surgical procedures. Its primary goal is to improve the overall appearance of the breasts by increasing their size and enhancing their shape.
The procedure can be performed using several techniques, including breast implants, transferring fat from another area of the patient’s body, or combining both methods in a procedure known as composite breast augmentation.
Women choose breast augmentation for different reasons. Some wish to add volume and improve the fullness of their breasts, while others want to correct breast asymmetry and achieve greater harmony in breast shape and size, resulting in a more balanced and aesthetically pleasing appearance.

Benefits of Breast Augmentation

Increased breast size: This may benefit women who have always felt self-conscious about the size of their breasts or who have experienced a reduction in breast volume after pregnancy or weight loss.
Improved breast shape: This may benefit women with asymmetrical breasts or those who have experienced changes in breast shape following pregnancy or weight loss.
Improved self-confidence: Many women feel more attractive, feminine, and confident after breast augmentation.
Better clothing fit: Fuller breasts may help clothes fit better and create a more attractive and balanced appearance.
Improved body proportions: Breast augmentation may improve the overall proportions of a woman’s body by creating better balance between the breasts, waist, and hips.

Breast Implant Options

Several breast augmentation options are available, including silicone implants, saline implants, and alternatives such as fat transfer. Each option has its own characteristics. It is important to consult a qualified plastic surgeon such as Dr. Ahmed El Sayed, Consultant Plastic Surgeon and holder of a PhD in Plastic Surgery from Kasr Al Ainy Faculty of Medicine, to determine which option is most suitable for you.
Silicone implants: Silicone implants are filled with a cohesive silicone gel that closely resembles the feel of natural breast tissue. They are pre-filled and available in a variety of sizes and shapes. Silicone implants generally provide a more natural appearance and feel compared with saline implants.
Saline implants: Saline implants are filled with sterile salt water and are available in different sizes and shapes. They are inserted while empty and then filled once they are correctly positioned, making them a good option for women with smaller breast tissue. They are also less expensive than silicone implants.
Fat transfer: This procedure uses the patient’s own fat to enhance the size and shape of the breasts. Fat is removed from another area of the body through liposuction, prepared, and then carefully injected into the breasts. It's a less invasive procedure as it doesn't require incisions but is performed with a single injection. Fat transfer can provide a natural appearance and feel, but it usually cannot achieve the same increase in breast size as silicone or saline implants.
Breast implants are available in two main shapes: round and teardrop-shaped. Round implants tend to create a fuller and more rounded appearance, while teardrop-shaped implants are designed to resemble the natural contour of the breast.

Choosing the Right Breast Implant Size

Choosing the appropriate breast implant size is an essential part of breast augmentation, as it has a significant effect on the final result. Dr. Ahmed El Sayed, Consultant Plastic Surgeon and holder of a PhD in Plastic Surgery from Kasr Al Ainy Faculty of Medicine, considers several factors when selecting the most suitable implant size.
Body proportions: The implant size should be proportionate to your body type. For example, excessively large implants may look unnatural on a woman with a smaller body frame. On the other hand, very small implants may not provide a noticeable or satisfactory result for a woman with a larger frame.
Current breast size: Your existing breast size and the amount of natural breast tissue also help determine the appropriate implant size. Women with limited natural breast tissue may require a large implant to achieve their desired result. Women who already have sufficient breast tissue may achieve their goal with a smaller implant.
Lifestyle and activities: Your lifestyle and the physical activities you regularly perform should also be considered. For example, very large implants may be less comfortable or practical for women who participate in sports or high-impact activities.
Personal goals: Your desired result and personal preferences play an important role in selecting the appropriate implant size. A smaller implant may be more suitable if you prefer a subtle and natural-looking enhancement. Larger implants may be considered if you want a more noticeable change.

Breast Implant Placement: Under or Over the Muscle?

The position of the breast implants is an important consideration in breast augmentation surgery. The two most common options are placing the implant underneath the chest muscle or above it.

Implant Placement Under the Muscle

Provides greater tissue coverage, which may produce a more natural-looking breast.
May reduce the risk of implant shrinkage, a condition where scar tissue forms around the implant, causing it to harden and potentially change shape or position.
May reduce the risk of visible implant rippling and improve the accuracy of mammography in some cases.
May be associated with a longer recovery period because the muscle needs time to heal and adapt to the implant.
May cause more discomfort during the early recovery period due to stretching and pressure on the muscle.
May require additional restrictions on physical activity during recovery.
In some patients, movement of the chest muscle may temporarily affect the appearance or position of the implant.

Implant Placement Over the Muscle

Usually involves a faster recovery because the chest muscle is not surgically affected.
May be a simpler option for suitable patients who have enough natural breast tissue to cover the implant.
May provide greater breast projection.
May appear less natural in women who do not have enough breast tissue to cover the implant.
May increase the risk of implant shrinkage and rippling.
May make mammography more challenging because the implant can obscure some breast tissue.

Who Is a Suitable Candidate for Breast Augmentation?

The patient should have reached adulthood and her breasts should be fully developed. This helps ensure that the body has reached maturity and that the surgical results are more stable.
The patient should be in good general health and should not have any medical conditions that may increase the risks of surgery or interfere with recovery.
A body mass index of less than 30 is generally preferred to help reduce the risk of complications and support a smoother recovery.
Women considering breast augmentation through fat transfer must have a sufficient amount of donor fat available for liposuction. The procedure involves transferring fat from other areas of the body to the breasts, so an adequate amount of fat must be safely available.
Candidates should have a genuine desire to improve the appearance of their breasts because of concerns regarding their size, shape, symmetry, or volume.
Candidates should have realistic expectations regarding the procedure and its possible results.

 Breast Augmentation with Implants

Step 1: After administering anesthesia, an incision measuring approximately 4–5 centimeters is usually made beneath the breast.
Step 2: A carefully designed pocket is created either above the chest muscle, under the breast tissue, or beneath the chest muscle. The pocket is created according to the dimensions of the selected breast implant.
Step 3: The selected breast implant is inserted and positioned inside the pocket.
Step 4: Dr. Ahmed El Sayed closes the incision using dissolvable sutures to support safe healing. Therefore, the sutures usually do not need to be removed.
Step 5: The patient wears a specially designed supportive bra after breast augmentation with silicone implants.

When Will the Final Results of Breast Augmentation Appear?

The final results of breast augmentation surgery with Dr. Ahmed El Sayed, Consultant Plastic Surgeon and holder of a PhD in Plastic Surgery from Kasr Al Ainy Faculty of Medicine, may take several months to become fully visible.
Most patients experience significant improvement within three to six months following surgery. During this period, swelling and bruising gradually subside, the implants settle into their final position, and the breasts develop a softer and more natural shape. Surgical scars also gradually fade and become less noticeable over time.
