Gynecomastia Treatment for Men

Gynecomastia surgery for men can be a life-changing procedure, helping improve your appearance and boost your self-confidence. Male breast enlargement involves the growth of excess breast tissue, which can cause significant embarrassment and loss of self-confidence.
Dr. Ahmed El Sayed, Consultant Plastic Surgeon and holder of a PhD in Plastic Surgery from Kasr Al Ainy Faculty of Medicine, may recommend treatment using VASER liposuction or removal of excess glandular tissue and fat that may accumulate in the chest area, depending on the severity of the condition. This helps contour, sculpt, and flatten the chest.

What Is Gynecomastia in Men?

Gynecomastia is a medical condition characterized by enlargement of breast tissue in males, usually resulting from an imbalance in hormone levels in the body. It can affect one or both breasts and may develop at any age, including childhood and puberty.
A variety of factors may contribute to this condition, such as hormonal imbalance, certain medications, drug use, obesity, liver disease, or genetic factors.
Gynecomastia often appears as a tender lump under the nipple, which may feel rubbery or firm. In some cases, it may be associated with nipple discharge. Although it is generally not a serious medical condition, gynecomastia can cause psychological distress in men.
Diagnosis involves a thorough examination and review of medical history. If necessary, additional tests such as blood tests or imaging may be requested to rule out other possible causes.
Treatment for gynecomastia depends on the underlying cause and may include medication to balance hormone levels, weight loss, or surgical intervention, such as male breast reduction, to remove excess tissue and restore a more masculine chest appearance.

Why Do Men Undergo Gynecomastia Surgery?

It is estimated that up to 60% of men experience some degree of gynecomastia. Although male breast enlargement is often associated with older men who are overweight, this condition is also common among younger men and teenagers who may develop prominent breasts.
However, male breast reduction should not be considered a weight-loss method or a treatment for obesity.

What Are the Benefits of Gynecomastia Surgery for Men?

The main benefit of male breast enlargement surgery is restoring a more masculine chest contour. By removing excess breast tissue and, in some cases, excess skin, the chest appears flatter and firmer, improving body image and self-confidence.
Physical comfort may improve after surgery. Men with enlarged breasts may experience discomfort during physical activities such as running or intense exercise, often because of friction. Surgery can help reduce this problem, making physical activities more comfortable and enjoyable.
After surgery, many men find that they have more clothing options. Previously, they may have avoided tight shirts or swimwear because of embarrassment. After surgery, they often feel more comfortable and confident in a wider range of clothing styles, improving their overall sense of style and comfort.
The psychological benefits are equally important. Men with gynecomastia may experience body-image concerns and may sometimes avoid social or physical activities to hide their condition. Successful surgery can improve self-confidence, social interaction, and overall psychological well-being.
The results of gynecomastia surgery are usually long-lasting, provided that a healthy lifestyle is maintained. This long-term improvement can give men peace of mind, knowing that they can enjoy their new chest contour for years to come.
Modern surgical techniques have also helped reduce scarring after gynecomastia surgery. Small incisions are used, resulting in less noticeable scars that fade over time and are usually placed in less visible areas.

Am I Suitable for Gynecomastia Surgery?

The best way to determine whether you are suitable for gynecomastia surgery is to consult a surgeon who specializes in male breast enlargement, such as Dr. Ahmed El Sayed, Consultant Plastic Surgeon and holder of a PhD in Plastic Surgery from Kasr Al Ainy Faculty of Medicine. Male breast reduction surgery usually involves liposuction, surgical excision, or both, to achieve a more masculine chest.
Many men may be physically fit but still suffer from puffy nipples or swollen breasts. When male breasts are prominent enough to cause emotional distress, this can be a valid reason to consider the procedure. There are three main categories of men who choose to undergo this surgery, including:
Younger men who feel embarrassed by the appearance of their chest, even if they are otherwise in good physical shape.
Older men in their late forties and above who want to remove excess fat and sagging skin.
Men who have lost a significant amount of weight and now have loose skin in the chest area. Diet and exercise cannot remove loose skin, so the best treatment may involve removing excess skin to tighten the chest. This may be combined with liposuction and gland removal to flatten the chest.

How to Prepare for Gynecomastia Surgery in Men

During the consultation, Dr. Ahmed El Sayed will take your detailed medical history and perform a physical examination to assess the severity of your gynecomastia. Your surgical goals and treatment expectations will also be discussed in detail.
Dr. Ahmed El Sayed will explain detailed preoperative instructions on how to prepare for surgery. These instructions include information about optimal nutrition, the importance of stopping smoking four weeks before the procedure, and fasting guidelines before surgery.
Smoking interferes with blood supply to the tissues and may increase the risk of wound-healing complications. Following all of your surgeon’s instructions carefully helps achieve the best possible results after gynecomastia surgery.

What Are the Details of Gynecomastia Surgery?

Gynecomastia surgery usually takes one to three hours, depending on the extent of surgical correction required.
The techniques used in male breast enlargement surgery depend on the underlying cause of the enlarged breast. Breast enlargement may be caused by excess fat, glandular tissue, or a combination of both.
Older men and those who have lost a significant amount of weight may also have excess skin, which can also be removed as part of the procedure.
VASER Liposuction: VASER liposuction uses high-frequency vibration, allowing fat to move more easily through the tissues and helping achieve more balanced results with minimal bleeding, less bruising, and faster recovery after the procedure.
Glandular Tissue Excision: Glandular tissue is carefully removed from beneath the nipple and the surrounding areas, as liposuction alone is not sufficient to treat true gynecomastia.
Excess Skin Removal: In some cases, the chest skin may not contract effectively around the new, smaller chest contour. This is especially common in men who have previously lost a large amount of weight. The best treatment is surgical removal of the excess skin and effective tightening of the remaining skin to create a firmer and more youthful-looking chest appearance.

What Can I Expect During Recovery After Gynecomastia Surgery?

Recovery can be smooth as long as you fully follow Dr. Ahmed El Sayed’s postoperative instructions.
After the procedure, a compression garment will be placed over your chest to protect the healing tissues. It is normal to feel mild discomfort during the first few days after surgery, and any discomfort can usually be controlled easily with pain medication.
It is also normal to experience bruising and swelling during recovery after gynecomastia surgery. Bruising may take up to two weeks to settle. Swelling can be improved by wearing the compression garment continuously for four to six weeks after surgery. Although most swelling will disappear during the first few weeks, it may take up to three months or more for the final results of gynecomastia surgery to become clear.
It is important to remain active during the early recovery period, and most men can return to work after one week. Your condition will be reviewed one week after surgery to check the incision and remove any stitches if needed.
Light exercise can be resumed after two weeks, although strenuous exercise should be avoided for six weeks after the procedure.
Excessive sun exposure to healing areas should be avoided to reduce the risk of hyperpigmentation. Sunscreen should be worn if you plan to spend a significant amount of time in the sun.

What Results Can I Expect After Gynecomastia Surgery?

The results of male breast enlargement surgery will be visible immediately after the procedure, although swelling may hide the final outcome for several weeks. The final results of gynecomastia surgery generally appear after three to six months.
Scars gradually fade and flatten over time, forming a faint white line. Dr. Ahmed El Sayed takes care to place the incisions as much as possible within the body’s natural skin folds to make them appear less noticeable.
